  12. I am looking for information from anyone using the 
  13. Advanced Linear Devices 4701 Quad Rail to Rail Opamp.

  15. I am interested in acquiring information on failure rates and modes.

  17. In our application, this component failed in 8% of all units.
  18. 1/2 of those failure were due to a misaligned pin which did not
  19. get soldered.  The second failure appears to be a loose wire bond
  20. on the V+ pin of the I.C.
  21.  
  22. Does anyone have any supporting/conflicting data on use of this chip?
