  23. Risks of Using the Norplant System
  24. ----------------------------------
  25.  
  26. 1. Irregular Menstrual Bleeding
  27.  Most women experience some change in their usual monthly pattern.
  28.  These menstrual irregularities vary from woman to woman and
  29.  include:
  30.   * Prolonged bleeding (more days than you usually experience),
  31.     commonly during the first months of use;
  32.   * Untimely bleeding or spotting between periods;
  33.   * No bleeding at all for several months; or
  34.   * A combination of these patterns.
  35.  It cannot be predicted what kind of change you may experience. If
  36.  increased frequency of bleeding occurs, the quantity of blood lost
  37.  is rarely enough to cause anemia, but there have been a few cases
  38.  that required treatment. The irregularities frequently diminish
  39.  gradually with continued use.
  40.  
  41. 2. Delayed Disintegration or Disappearance of Follicles
  42.  If follicles (eggs and their surrounding cells) in the ovary
  43.  develop while using the Norplant System, disintegration or
  44.  disappearance of the follicles is sometimes delayed, and the
  45.  follicles may continue to grow beyond the size they would normally
  46.  reach. These enlarged follicles may produce discomfort in some
  47.  women, although most users would not be aware of them unless they
  48.  were found incidentally on a physical exam. In the majority of
  49.  women, enlarged follicles will disappear on their own and should
  50.  not require major surgery. Rarely, they may twist or rupture so
  51.  that surgery is required. You should discuss this with your
  52.  health-care provider. 
  53. 3. Ectopic Pregnancies
  54.  Ectopic pregnancies (development of the fertilized egg outside the
  55.  uterus) have occurred among Norplant System users, although
  56.  clinical studies have shown no increase in the rate of ectopic
  57.  pregnancies per year among Norplant System users as compared with
  58.  users of no method or of IUDs. Symptoms of an ectopic pregnancy
  59.  include spotting and cramping pain, which usually begin shortly
  60.  after the first missed period. Contact your health-care provider
  61.  should you miss a period or experience abdominal pain. 
  62. 4. Risks Based on Experience with Combination Oral Contraceptives
  63.  Combination pills contain a progestin such as levonorgestrel and
  64.  an estrogen, another type of hormone. Some rare but serious side
  65.  effects have been associated with the use of the combination pill.
  66.  It is unknown whether the risks associated with combined
  67.  oral-contraceptive use may also be risks with a progestin-only
  68.  contraceptive like the Norplant System. Since the hormone is
  69.  released at a low rate, the Norplant System is expected to avoid
  70.  some of the side effects of combined oral contraceptives that not
  71.  only have higher progestin doses but also contain estrogen.
  72.   Risks Associated with Combination Oral Contraceptives Include:
  73.    a. Risk of developing blood clots - Blood clots and blockage of
  74.       blood vessels are the most serious side effects of taking the 
  75.       combination pill. In particular, a clot in the veins of the
  76.       legs can cause inflammation and risk of further clots, and a
  77.       clot that travels to the lungs can cause a sudden blocking of
  78.       the vessel carrying blood to the lungs, resulting in
  79.       respiratory collapse and even death. Rarely, clots occur in
  80.       the blood vessels of the eye and may cause double vision,
  81.       impaired vision, or even blindness.  
  82.    b. Risk of heart attacks and strokes - The combination pill may
  83.       increase the tendency to develop strokes (stoppage or rupture
  84.       of blood vessels in the brain), angina pectoris, or heart
  85.       attacks (blockage of blood vessels to the heart). Any of
  86.       these conditions can cause death or serious disability.
  87.       Smoking greatly increases the probability of suffering heart
  88.       attacks and stroke.  
  89.       Use of combination oral contraceptives together with
  90.       cigarette smoking greatly increases the risk of serious
  91.       adverse effects on the heart and blood vessels. 
  92.       This risk increases with age and with heavy smoking (15 or
  93.       more cigarettes per day) and is quite marked in women over 35
  94.       years of age who smoke. It is not known whether a similar
  95.       interaction occurs with the Norplant System. Therefore, women
  96.       who use the Norplant System should not smoke. 
  97.    c. Gallbladder disease - Combined-pill users probably have a
  98.       greater risk than nonusers of gallbladder disease. Since this
  99.       risk may be related to pills containing high doses of
  100.       estrogens, it may not be a concern for Norplant System users. 
  101.    d. Liver tumors - In rare cases, the combination oral
  102.       contraceptive can cause noncancerous but dangerous liver
  103.       tumors. These benign liver tumors can rupture and cause fatal
  104.       internal bleeding. In addition, a possible but not definite
  105.       association has been found with the pill and liver cancers.
  106.       However, liver cancers are very rare.
  107.    e. Cancer of the reproductive organs - There is, at present, no
  108.       confirmed evidence that combination oral contraceptives
  109.       increase the risk of cancer of the reproductive organs in
  110.       human studies. Recent studies have found no increased risk of
  111.       developing breast cancer in the overall population of users.
  112.       However, some of these same recent studies have found that
  113.       certain subgroups of oral-contraceptive users may be at an
  114.       increased risk, although no consistent pattern has been
  115.       identified. Some studies have found an increase in the
  116.       incidence of cancer of the cervix in women who use oral
  117.       contraceptives. However, this finding may be related to
  118.       factors other than the use of oral contraceptives.

  121. Side Effects of the Norplant System
  122. -----------------------------------
  123.  
  124.   The most frequently reported side effects are menstrual cycle
  125. irregularities. Such changes vary from woman to woman and may
  126. include: 
  127.   * Prolonged menstrual bleeding (more days than you would usually
  128.     experience), commonly during the first months of use;
  129.   * Untimely bleeding or spotting between periods;
  130.   * No bleeding at all for several months; or
  131.   * A combination of these patterns.
  132.  
  133.   It cannot be predicted before insertion of the Norplant System
  134. what kind of bleeding pattern you will have. Many women can expect
  135. an altered bleeding pattern to become more regular after 9 to 12
  136. months. Despite the increased frequency of bleeding in some women,
  137. the monthly blood loss is usually less than normal menses. In fact,
  138. in some studies, patient blood counts have improved. 
  139.   Contact your health-care provider if you experience heavy
  140. bleeding. If you have normal cyclic periods and then miss a period,
  141. a pregnancy test should be obtained. 
  142.   In clinical studies, women using the Norplant system have
  143. complained about the following conditions, which are possibly
  144. related to the Norplant System:
  145.   * Headache;
  146.   * Nervousness;
  147.   * Nausea;
  148.   * Dizziness;
  149.   * Enlargement of the ovaries and/or fallopian tubes;
  150.   * Dermatitis (inflammation of the skin);
  151.   * Acne;
  152.   * Change of appetite;
  153.   * Weight gain;
  154.   * Mastalgia (breast tenderness);
  155.   * Hirstutism (excessive growth of body or facial hair) or
  156.     alopecia (hair loss);
  157.   * Discoloration of the skin over the site of implantation
  158.     (usually reversible).
  159.  
  160.   Preexisting conditions of acne or excessive growth of body or
  161. facial hair could also be worsened. Occasionally, an infection may
  162. occur at the implant site, or there may be a brief incidence of
  163. pain or itching. Removals may be more difficult than insertions in
  164. some cases. 
  165.   An enlargement of ovarian follicles, detectable during a physical
  166. examination, sometimes occurs in Norplant system users. The
  167. enlarged follicles usually disappear on their own without surgical
  168. intervention, but in rare instances they may twist or rupture, so
  169. that surgery is required. 
  170.   There are a number of other complaints reported by Norplant
  171. System users or discovered by health-care providers, but an
  172. association with the Norplant System has been neither confirmed nor
  173. refuted:
  174.   * Breast discharge;
  175.   * Cervicitis (inflammation of the cervix, detected by physician);
  176.   * Muscle and skeletal pain;
  177.   * Abdominal discomfort;
  178.   * Leukorrhea (whitish discharge from the vagina and uterine
  179.     cavity); 
  180.   * Vaginitis (inflammation of the vagina).

  183. Side Effects Based on Experience with Combination Oral
  184. Contraceptives
  185. --------------

  187.   As with the risks associated with taking combination oral
  188. contraceptives, it is unknown whether the possible side effects
  189. listed below would also occur with a progestin-only contraceptive
  190. like the Norplant System.
  191.   a. Contact lenses - If you wear contact lenses and notice a
  192.      change in vision or an inability to wear your lenses, contact
  193.      your doctor or health-care provider.
  194.   b. Fluid retention - Combination oral contraceptives may cause
  195.      edema (fluid retention) with swelling of the fingers or ankles
  196.      and may raise your blood pressure. If you experience fluid
  197.      retention, contact your doctor or health-care provider. 
  198.   c. Melasma - A spotty darkening of the skin is possible,
  199.      particularly of the face.
  200.   c. Other side effects - Other side effects that have been
  201.      observed with the use of combination oral contraceptives, but
  202.      not necessarily the Norplant System, include depression and
  203.      rash.
