Those interested in this area will want a copy of my PhD thesis, which does
an extensive analysis of how people use Unix (but not quite looking at the
"problems" with Unix, which I think are pretty obvious.)

It will be coming out in January as a book published by Cambridge University
Press titled
 "The User As Toolsmith: The Use, Reuse and Organization of
  Command-based Interfaces"
