  3.    reporting it.  This allows us to track the spread of PC viruses and
  4.    hopefully determine their source.

  6.  o If you encounter a virus that IM doesn't recognize, please save at least
  7.    one infected floppy or file and report this to us.  We will determine if
  8.    it's a virus and send an updated version of IM at no charge.

  27.    Your name and telephone #, plus a good time to call (or your address if you
  28.    prefer not to be called).

  30.    The number of PCs affected.

  32.    Describe any damage or loss of data:

  34.    Describe any noticable effect of the virus on your PC.

  37. To report a virus which IM doesn't recognize by name (an unknown virus):

  39.    Serial number and version number of Integrity Master (You are NOT
  40.    required to already be a registered user to report a virus)

  42.    Your name and telephone # (optional), plus a good time to call.

  44.    Your mailing address
  45.  
  46.    The number of PCs affected.

  48.    Describe and damage or loss of data:

  50.    Describe any noticable effect of the virus on your PC.

  52.    What type of PC do you have?

  54.    What version of DOS?

  56.    Please mail an infected file or preferably entire infected floppy.  Do
  57.    NOT delete any files from the floppy, if the virus infects diskettes.
  58.    We will extract and analyze the virus.  Your files will be kept confidential
  59.    and will be either destroyed or returned to you upon your discretion.  As
  60.    a reward for your trouble, we will send you the latest version of Integrity
  61.    Master.
