  4. INSTRUCTIONS FOR FILLING OUT THIS FORM:  I sincerely want to resolve any
  5. problems you encounter with my doors but to do this I must have accurate
  6. and concise information on the problem. Too many people call me and expect
  7. me to resolve their problem on nothing more then "it does not work". That
  8. simply will NOT work to resolve the problem.  Before you fill this form out
  9. I request that you complete the B4UASK.DOC checklist to make sure you have
  10. not forgotten to do something in the setup of the game.  If that does not
  11. alleviate the problem then the next step is to completely fill this form out
  12. AND package it up with any error logs, batch routines and the CTL file
  13. and either freq it to me at my FIDO address OR upload it to my BBS. Please
  14. note that I usually do not respond to inquiries unless it is either through
  15. my BBS or through the FIDO netmail system.  If you want a reply then you
  16. must either log onto my BBS OR you must poll back 48 hours later for my
  17. reply to your message. I habitually monitor ALL door related echos on the
  18. FIDO net so that is also a good place to post your questions (and cheaper in
  19. the long run, if you don't mind the delays). My support BBS can be reached
  20. at (209) 276-3657 (2400 to 28800 baud).  My FIDO number is 1:205/65
