- Here in Pittsburgh, there is one EM resident from the Univ of Pgh
- Medical Center that handles requests for orders first, and responds to
- most cardiac arrests and other critical patients, if readily proximal.

- If he/she is tied up, then a "faculty backup" physician in the UPMC ER

- handles command requests, but doesn't respond to the scene.

- If the medics encounter a situation where the patient is adamantly
- refusing transport, or where the medics do not think a medical emergency

- exists, they can consult a physician to obtain a release from the
- scene.

- This is done with many "samsonite" calls, where the patient is packing
- their own bag for a direct admission to the hospital. If the MD
- approves, the medics leave and refer the patient to a private service.
