  10. Here in Pittsburgh, there is one EM resident from the Univ of Pgh 
  11. Medical Center that handles requests for orders first, and responds to 
  12. most cardiac arrests and other critical patients, if readily proximal.

  14. If he/she is tied up, then a "faculty backup" physician in the UPMC ER
  15.  
  16. handles command requests, but doesn't respond to the scene.

  18. If the medics encounter a situation where the patient is adamantly 
  19. refusing transport, or where the medics do not think a medical emergency
  20.  
  21. exists, they can consult a physician to obtain a release from the
  22. scene.

  24. This is done with many "samsonite" calls, where the patient is packing 
  25. their own bag for a direct admission to the hospital. If the MD 
  26. approves, the medics leave and refer the patient to a private service.
