Look in the back of any bicycling, running orhiking/backpacking magazine. There are always ads for this stuff. That's where I got mine. I have some dogtags on chains that I put through the little hole on the zipper mechanism when I wear my jerseys.
I also have one that has a vecro wrist strap . I use this because in the summer if I go out for less than an hour I usually wear a T-shirt and not a jersey.
A friend of mine has the kind that you can attach to. your shoe using the laces.
On my dog tag I have my name, my home phone #, my HMO phone #, my HMO medical record number, my blood type and my allergies.
