  5. I, __________, desire to make aware that after mature reflection, 
  6. and, being aware of the right under the law to decline life-
  7. sustaining treatment, that I wish, should I ever be unable to
  8. make decisions for myself concerning my medical treatment that 
  9. I receive life sustaining treatment even after a terminal 
  10. diagnosis, even if the life prolonging treatment will delay
  11. the natural process of dying.

  13. I have previously made a "living will" or other document 
  14. expressing a desire contrary to that specified herein, and by
  15. this document I herewith revoke the same.
