  10.     If the time comes when I am incapacitated to the

  12. point where I can no longer actively take part in decisions

  14. for my own life, and am unable to direct my physician as to

  16. my own medical care, I wish this statement to stand as a

  18. testament of my wishes. I ______________ request that I be

  20. allowed to die and not be kept alive through life support

  22. system if my conditionis deemed terminal.

  24. I do not intend any direct taking of my life,
