The Media Loading page lets you do the following:
Control how QCD loads audio files into the playlist
Set up a folder to use as a "watched folder" :audio files added or removed from that folder are automatically updated in the playlist.
To display the Media Loading page:
In the top left corner of the base skin , click the Main Menu button, choose Preferences, and then click Settings then Media Loading.

Set to have files added to the top of the playlist instead of appended to the end. |

Set to have QCD appear on top of other windows whenever you load new files. |

Append files to playlist by default (play otherwise) |
When you launch files by double-clicking them in Windows Explorer, by default they will be played in QCD. Set this option to have the files added to the playlist instead. |

Folder Mirroring |
Note: Folder Mirroring will watch a folder and update the playlist based on any changes to audio files in that folder. |
