Day 128 - 24 May 95 - Page 31
Regional Managers' meetings. It was a regular meeting that
was scheduled, but I think it was probably before we were
into minutes.

Q. Would that have been discussed at minutes of the Health and
Safety Management Task Force?
A. It might not have been, in that once we had agreed with
Andrew we were going to change it, we moved things pretty
quickly. The problem with the Task Force meetings is they
are only once a quarter, and quite often there are things
that come along that just need to be actioned in between
the meetings.

Q. Was it discussed at board level, do you know?
A. I doubt it was discussed at board level, but the three
Regional Managers were all Vice Presidents at the time, so
a part of that executive team anyway.

Q. Just something that I noticed yesterday, perhaps you can
confirm it: Notifiable accidents, that does not include
broken bones in the hands or feet, does it?
A. It does not in a major category, no, but if a person
was off for more than three days then, yes, it would be
reportable, which the likelihood for an injury like that is
they would be off for more than three days.

Q. You said that any reported accidents are investigated by
the Company. In the cases where that happened, would there
be a report prepared by the Company, either by yourself or
somebody else in your department, on that accident?
A. Part of the accidents investigation format is that the
Supervisor or Restaurant Manager, depending on the severity
of accident, takes statements and attaches them to a
covering sheet which just goes through the main points of
accident. So, if you are asking me is there any
documentation, then, yes, those accidents investigation
reports are kept on file, and are monitored by my people as
well.

Q. How long has that been done for?
A. As we touched on yesterday, it was early 1992 when we
first started asking for a formal investigation of
accidents.

Q. Can I just ask, yesterday you said: "We do not do any
statistics from incident report forms, but of course the
restaurants do fill them in for minor accidents as well".
When you said "minor accidents", what did you mean by that?
I mean, would not fill out incident report forms for
burns and cuts, would they?
A. Some of them do do.

Q. Some of them do; what, they might report every single
accident that was recorded in the accident book?
A. Some of them are extremely zealous with their paper
work and would fill out an incident report form for almost
any cut or burn that happened in the restaurant.

Q. But that is not the common practice?
A. No, the common practice is to fall in line with the
