7 A. If there has been an accident to an employee.

10 A. It is my belief it is only for employees.

12 Q. The incident report forms, they also get filled out when an
13 employee has an accident?
14 A. Yes, they will be do.

16 Q. They are not the same as the RIDDOR -- what is the word? --
17 watershed that you have to be more than three days in
18 hospital or something like that, or it has to be a broken
19 bone?
20 A. Yes.

22 Q. When does an incident report form get filled out for an
23 employee accident?
24 A. It would most likely correspond to RIDDOR, unless it
25 was something out of the ordinary that happened.

27 Q. So any accident less than three days hospitalisation or a
28 broken bone, head office does not get informed of it and
29 the information is not collated?
30 A. It would be recorded in the accident report book in the
31 restaurant available for anyone to check whenever they
32 like. If there is something notable about the incident
33 report form it will be filled in. If anything we fill in
34 too many incident report forms. When they get to head
35 office there are ones that may not be necessary.

37 Q. You said that if there is something notable it will go in
38 an incident report form. Then you said that the level is
39 equivalent to the RIDDOR level?
40 A. No. Try to clarify?

42 Q. I want to know, roughly, when does an incident report form
43 relevant to an employee, at what level is the manager
44 expected to start taking that as a significant accident?
45 A. I will try to explain. With RIDDOR accidents
46 notifiable in the incident report form will be filled in as
47 a matter of course and people within the company informed.
48 If a crew member just cuts their finger, it is a minor
49 incident, that would just go in the blue book. If perhaps
50 something between those two happens, it need not be
51 informed to RIDDOR, it is not legally something we should
52 do, yet it is maybe a bit more serious than a cut finger
53 and an incident report form may be filled in.
