Day 127 - 23 May 95 - Page 43
A. That was done straightaway, yes.

Q. Did Keith Smith, which is the next one, compile a step by
step guide to wiring a plug?
A. He did, yes, which was used as part of the revised AEC.

Q. Was it laminated and kept in the equipment service history
file?
A. I am not sure that that happened.

Q. Last one on that page, "Keith Smith, Paul Brittas and Frank
Frost developing training for all shift running managers
working with Jasper Mordsley". Who is he?
A. Jasper is the market manager in the north east.

Q. "This pilot will be assessed to determine the benefit and
to ascertain whether the information will be better
incorporated into e.g. the BAC, the FMC or the AEC." What
happened there?
A. The pilot was not progressed because the stamps we took
was doing less work instead of getting more people to do
more work. Although it was run in the north east, we did
not run it nationwide.

Q. Getting less people was safer than training more people?
A. Yes.

Q. On page 10, the first recommendation, "Suggestion: The
training group is to consider how to improve first aid
training and whether increased training of personnel to an
appointed person level one day course is a practical
option", and I think you have told us about that?
A. Yes.

Q. Then finally at 6.4, "health and safety management plan",
"A health and safety management plan is being formed by
the health and safety executive recommendations to improve
compliance with the existing and new legislation and to
incorporate lessons learned from this accident."

Later on I am going to ask you to look very briefly at the
HSE report, Mrs. Barnes. It made 23 recommendations, if
you recall. Did McDonald's implement any of those
recommendations?
A. Yes, we implemented them all and they were all
implemented within 18 months of that December meeting.

Q. Did that cost money?
A. Yes. Certainly ----

Q. And effort?
A. Certainly a lot of effort and a lot of people's time,
but we all felt it was worth it.

Q. Then, Mrs. Barnes, further on -- go past your two
appendices, please -- we get to a page headed "Harborn"; do
you see that?
A. I do.

Q. Which is a place, not a person?
