     4        A.  The incident report forms are used by every restaurant,
     5        whether they are franchisee or company owned.  I believe
     6        East Kilbride is actually a franchisee restaurant.

     9        A.  I believe East Kilbride, the one with the sticker on
    10        the front, is a franchisee restaurant.  So it is quite
    11        likely that it was done -----

    15        A.  Yes.

    17   Q.   Is that a franchise or is it company owned?
    18        A.  That one is company owned.

    20   Q.   One can see, I hope, they are in fact the same form.  But
    21        the licensees would get the forms from the company, would
    22        they?
    23        A.  Yes, they are available from our distribution centre.

    25   Q.   Where do the licensees send the forms?
    26        A.  That -- I would suspect a copy to Head Office to their
    27        field consultant.

    30        A.  Yes.

    32   Q.   What happens to these incident report forms when they are
    33        received at a higher level in the company?
    34        A.  Well, they are copied to interested departments.  So,
    35        for example, the ones to do with accidents would come to
    36        us.  If it is a service complaint, a foreign object like a
    37        bone in a chicken, then that would go to operations and to
    38        quality assurance.  There are administration people in each
    39        office who would copy them and send them out appropriately.

    41   Q.   Have you got the East Kilbride one there, and tell us
    42        whether this is the sort of thing you -- just the first
    43        couple of ones.  The first one is -- the number is 425; the
    44        date 20th December 93, just before Christmas, 25 past 6 in
    45        the evening, "Maureen, the lobby hostess, moved ladders on
    46        lobby, lost grip and they hit customer on head."   That
    47        means the ladders, I take it?

    50   Q.   Then turn over the page, this is on the next day, the 11th 
    51        December 93, a piece of bone found inside chicken Nuggett,
