    12        of the chart, but looking at only -- is it headed "status
    13        report"?

    16   Q.   And are there five columns?
    17        A.  Yes.

    19   Q.   "Dangerous behaviour/condition, remedial action ...(reads
    20        to the words)... date for completion, those involved"?
    21        A.  That is right, yes.

    23   Q.   What, overall, does this document tell us?
    24        A.  Well, it tells us that by the beginning of December we
    25        had really made a big impact on those areas that we had
    26        highlighted we wanted to address.  A lot of it builds on
    27        efforts that we had in place before the accident,
    28        particularly with the work that was already going on with
    29        regard to checks on portable equipment and fixed system
    30        checks as well.

    32   Q.   So it is, in effect, a progress report; is that right?
    33        A.  It is, yes.

    35   Q.   One sees it is, as you rightly pointed out, dated 2nd
    36        December, whereas your original report with its memo was
    37        4th November.
    38        A.  That is correct.

    40   Q.   So this is what you have been able to achieve in the
    41        intervening months; is that right?
    42        A.  Yes.

    44   MR. RAMPTON:  My Lord, apart from possibly one or two little
    45        bits and pieces, that is the substance of the evidence in
    46        chief.  I thought it important to go through that document.

    53        October 1992, I was asking you about McDonald's structures,
    54        systems, procedures and attitudes to health and safety in
    55        general.  I would like to ask you, if I may, now about, in
    56        particular, training in relation to health and safety.  For
    57        this purpose you will need volume XIII which you already
    58        have, XI which you already have as well, and another one
    59        called X which is the Operations Training Manual.
