     5        column, will each half be about A4 then?  Because if
     6        Mrs. Brinley-Codd could fold it down the middle gap, it can
     7        be quite readily photocopied here and now on the machine.

     9        because that is the extract from the Personnel and
    10        Procedures manual.

    18        nevertheless, if you could just make about four or five
    19        copies of both sides.  We will carry on with the evidence

    22   MR. RAMPTON (To the witness):  Please turn to 766, Mrs. Barnes,
    23        which is a memo dated 29th June 1992 from Margaret Harmer
    24        to Restaurant Managers, copied to you amongst others.  She
    25        writes:  "Due to the increasing amount of paper work and
    26        documentation on health and safety, I write to advise you
    27        that you should set up a restaurant health and safety file
    28        to hold relevant information".  Then she says they have
    29        drawn up an index which I think one can see on the next
    30        page, 767.

    32        That was, it appears, a new measure, was it, Mrs. Barnes?
    33        A.  It was and it is a measure that we have continued.

    35   Q.   What goes in the health and safety file?
    36        A.  Very much what is written there on the index.  It is
    37        really -- it is live documentation.  So, it would be a copy
    38        of an FT5 which is a form that we use to report RIDDOR
    39        accidents, or it would be the health and safety memos so
    40        that they are kept all in one place so they are easy to
    41        refer to, or it could be Environmental Health Officer
    42        correspondence from their visits, or, you know, as you can
    43        see, lift certificates and so forth.

    45   Q.   All those categories of documents which we can see written
    46        on that page -- you will have to forgive me, Mrs. Barnes,
    47        I am going as quickly as I possibly can for Mr. Morris'
    48        sake -- would be expected to go into the health and safety
    49        file, yes?
    50        A.  Yes.

    52   Q.   Including anything under 14 "miscellaneous", I 
    53        assume, which was relevant to health and safety?
    54        A.  Yes.  We have extended the index a little since.  There
    55        are fewer in the Miscellaneous.

    58        order, but it is easier to go through them in page order
    59        than to dot about.  768.
