10 A. It is on the Manager's notice board in every
11 restaurant, so it is kept in their office, close to the
12 phone.

15 A. Yes, well, it is actually in that format at the
16 moment. I am afraid we forgot to laminate it. The next
17 print, we will have it laminated.

20 "In the event of a electric shock remember to", then the
21 first thing is, "administer First Aid". Would it not be
22 important to inform management and employees that actually
23 they should make sure the current is still not passing
24 through the person who has suffered the shock?
25 A. You are thinking that, in going to give First Aid that
26 might happen. Any trained First Aider would be trained in
27 that anyway. I understand why you put it.

58 A. There was not a systemised policy for operations people
59 to investigate it. They were investigating in more of an
60 ad hoc way, usually using security people to take the
