4 Q. This calls itself "Accident and Dangerous Occurrence
5 Procedure Check List". Where if I am looking for it do
6 I find this in the McDonald's system?
7 A. It is usually on the notice board in the manager's
8 office.

10 Q. In the manager's office?
11 A. In the manager's office.

13 Q. Not on the crew notice board?
14 A. No. It is designed as a check list for managers so
15 that if they have an accident they can just quickly look
16 through what they need to do.

18 Q. It says give: "First Aid Immediately. Contact your
19 supervisor and regional health adviser." Who exactly is
20 the regional health adviser?
21 A. It is the regional health and safety adviser.

23 Q. He or she is one of your three subordinates?
24 A. Yes.

26 Q. " --- /human resources if an accident is reportable". Then
27 it tells him or her what are the reportable accidents,
28 including at the bottom of the box: "If an accident results
29 in an employee being off sick for over three days whether
30 or not scheduled to work." Then the red sign I assume means
31 telephone?
32 A. It does, yes.

34 Q. Yes. "Your local environmental health department must be
35 informed immediately by phone in the event of a major
36 accident." Then it says: "Regional and safety
37 advisers/human resources will notify both major and
38 three-day accidents to EHOs." Then in big black capitals:
39 "IF IN DOUBT CONTACT YOUR REGIONAL HEALTH AND SAFETY
40 ADVISER OR HUMAN RESOURCES." What sort of case, from your
41 experience, would give rise to a doubt and prompt the
42 manager to ring up the health and safety adviser and say:
43 "Is this reportable or is it not? What shall I do about
44 it?"
45 A. Something like if somebody fell over and twisted their
46 knee and obviously were in some pain at the time and
47 perhaps had to go home from the shift, they were not quite
48 sure whether they were going to be away for the three days.

50 Q. So the manager would not wait to see they were going to be
51 in hospital for 24 hours or more; they would not wait to
52 see whether they were away for three days?
53 A. Some would. Some ring up and ask about everything. We
54 would rather they rung if in doubt than not ring and
55 perhaps forget to contact us.

57 Q. Then it says: "Complete accident book for every employee
58 accident." Does that include all the little cuts and burns
59 stubbed toes?
