outline that was on the accident check list, the coloured
one we went through yesterday, in terms of it being people
who lose time off work, a reportable accident or a customer
       accident.

  Q. Can I just ask about this coloured form?
       A. Yes.

  Q. Who was this form directed at?
       A. It is on the Manager's notice board in every
restaurant, so it is kept in their office, close to the
       phone.

MR. JUSTICE BELL: Is that the one you said was laminated?
       A. Yes, well, it is actually in that format at the
moment. I am afraid we forgot to laminate it. The next
print, we will have it laminated.

MS. STEEL: In the box in the right hand bottom corner it says:
"In the event of a electric shock remember to", then the
first thing is, "administer First Aid". Would it not be
important to inform management and employees that actually
they should make sure the current is still not passing
through the person who has suffered the shock?
       A. You are thinking that, in going to give First Aid that
might happen. Any trained First Aider would be trained in
that anyway. I understand why you put it.

  Q. The first thing would be to make sure the equipment was
       switched off and -----
       A. Isolated.

  Q. Yes.

MR. JUSTICE BELL: We can break off there for the middle of the
       day, if it suits you?

MS. STEEL: OK. I was going to move on to something else.

MR. RAMPTON: Would your Lordship like us to feel the texture?
This is the original one. It has a kind of lamination on
it, I suppose. (Handed). It is not quite the same as the
       coloured copies we have.

MR. JUSTICE BELL: "Laminated" means there is more than one
       thickness of it?
       A. Yes, it would be harder plastic. It is one of the
things the restaurants have requested for the next print
       run.

                      (The short adjournment.)

MS. STEEL: The memo that was referred to yesterday about "It is
now company policy to formally investigate all accidents,
which, by law, we have to report to the EHO". You said
before that date you did not investigate all of the
       accidents; is that right?
       A. There was not a systemised policy for operations people
to investigate it. They were investigating in more of an
ad hoc way, usually using security people to take the
