Day 084 - 07 Feb 95 - Page 45

anything which is considered to be of significance by the
       local management. Is that correct?
       A. We do not have a RIDDOR form, if that is what you
       think.

  Q. The RIDDOR comes in when what, then?
       A. If there has been an accident to an employee.

  Q. Only for employees?
       A. It is my belief it is only for employees.

  Q. The incident report forms, they also get filled out when an
       employee has an accident?
       A. Yes, they will be do.

  Q. They are not the same as the RIDDOR -- what is the word? --
watershed that you have to be more than three days in
hospital or something like that, or it has to be a broken
       bone?
       A. Yes.

  Q. When does an incident report form get filled out for an
       employee accident?
       A. It would most likely correspond to RIDDOR, unless it
was something out of the ordinary that happened.

  Q. So any accident less than three days hospitalisation or a
broken bone, head office does not get informed of it and
       the information is not collated?
       A. It would be recorded in the accident report book in the
restaurant available for anyone to check whenever they
like. If there is something notable about the incident
report form it will be filled in. If anything we fill in
too many incident report forms. When they get to head
office there are ones that may not be necessary.

  Q. You said that if there is something notable it will go in
an incident report form. Then you said that the level is
       equivalent to the RIDDOR level?
       A. No. Try to clarify?

  Q. I want to know, roughly, when does an incident report form
relevant to an employee, at what level is the manager
expected to start taking that as a significant accident?
       A. I will try to explain. With RIDDOR accidents
notifiable in the incident report form will be filled in as
a matter of course and people within the company informed.
If a crew member just cuts their finger, it is a minor
incident, that would just go in the blue book. If perhaps
something between those two happens, it need not be
informed to RIDDOR, it is not legally something we should
do, yet it is maybe a bit more serious than a cut finger
and an incident report form may be filled in.

  Q. You care about the health and safety of employees, do you
       not?
       A. I do indeed.

  Q. You have been a manager yourself for a substantial time?
       A. I have.
