     5   MR. JUSTICE BELL:  Just read that through to yourself.  Assume
     6        that the longhand on it is accurate because we have no
     7        other information about the publication.
     8        A.  OK.  I have read that through now.

    10   Q.   Does that jog your memory in any way?
    11        A.  It does a little bit.  I do not think it was in
    12        Shrewsbury.  What jogs my memory is the electronic beeps.

    14        that we had been discussing the removal of chicken and
    15        looking at ways that we may improve what we are doing in
    16        the kitchen, and this may well have been where this came
    17        from.

    19   MS. STEEL:   What exactly have you been discussing in relation
    20        to that?
    21        A.  Well, we have been, as always, looking at ways that we
    22        can improve our systems.  I believe it came, someone
    23        suggested that the sound of the alarms in the kitchen when
    24        a product was cooked could be confusing, and it seemed to
    25        be maybe something in, that if we had different noises on
    26        each of the stations ----

    28   MR. JUSTICE BELL:  Someone may hear a beep go off and think it
    29        was the chicken they are cooking when it is something else?
    30        A.  That is right.  I think this was brought to me by Jill
    31        that this may have been a better idea.  I remember
    32        discussing it with her.  There may well be some merit in
    33        it, but my thoughts on that was that we have lights that
    34        flash at the same time as the beep going off, so it would
    35        be a visual reminder as well as an audible reminder, so if
    36        you hear the noise and see the light you know which the
    37        basket that refers to.  However, I think it may be of some
    38        merit to look into further; it may be yet another of the
    39        many safeguards to cooking products in the kitchen.

    41   MS. STEEL:  Did Jill mention any specific incident in relation
    42        to this problem?
    43        A.  I am a bit foggy on the background.  I get a report of
    44        current things that are going on.  I cannot remember this.
    45        Maybe it has not come through to me yet as an issue.
    46        I know she has consulted with me on it.

    48   Q.   She did not say about any specific incidents to you?
    49        A.  No.  If it was in Shrewsbury then Mark Hatherway is the
    50        regional health and safety officer for that area.  He would 
    51        have dealt with it in the first instance, referred to Jill 
    52        if it needed further investigation.  They are perfectly 
    53        capable of covering alleged food poisoning incidents.  They
    54        fully understand our system.  If there is anything
    55        particularly unusual then they may come and refer to me for
    56        it.  I believe that may well have been what happened in
    57        this instance.  I am not sure if it was Shrewsbury.
