Day 127 - 23 May 95 - Page 42

  Q. Then there is a recommendation for revising the procedures
for cleaning the filtering machines; do you see that?
       A. I do.

  Q. "Aim is to better specify procedures so that there is
easier access to plugs and minimal use of water". What
       happened there?
       A. That was revised by Richard Rowley, our equipment
manager, although we still need to continue to use water.
Steps were taken to make sure that cables were long enough
or plugs were close enough, that we would never again get
the taut cable contribution to an accident that happened at
       Manchester.

  Q. The next one, "Richard Rowley is working with Ridling to
address the weaknesses in the machine highlighted by the
EHO" -- that is Mr. Shirkie. Did Mr. Rowley have any
success in that exercise, do you know?
       A. He did. We had retrofits done to the switching
mechanisms in all of the existing filtering machines and
had the design changed. We were not able to find a UK
based organisation that were able to sanction what we had
done in terms of giving us some sort of certification.

  Q. That is the bit you were looking at?
       A. Yes, that is correct. But since then EC legislation
has come in which self-certifies the manufacture -- they
self-certify against certain criteria that equipment is
       safe.

  Q. The last paragraph on that page, "Mr. Rowley is
coordinating setting up the audits"; do you see that?
       A. I do.

  Q. Is that done?
       A. Yes, it has been done. The audit that you saw before,
the electrical audit, was amended to include that.

  Q. So there was in place already an audit and it was added to?
       A. Yes.

  Q. Then on the next page 9, "training", "The AEC", you
thought, "ought to be revised to include more emphasis on
telling people who had been on AEC that they must inform
non-qualified personnel that they should not attempt
...(reads to the words)... additional information on
wiring a 13 amp plug." Were those things done?
       A. They were both done, yes, right down to producing
circuit boards and part piece of equipment that the
students on the AEC would have to show that they could work
       on properly.

  Q. Not just drawings?
       A. No, not just drawings. It was actually a little light
circuit board that they have to show they could work
       properly.

  Q. The next one, "The floor managers' post package to be
revised to better specify what should or should not be
       done" etc. Was that done?
