Day 195 - 04 Dec 95 - Page 22
was not hygienic since the temperature of the storage
cabinet was not high enough, specially when defrosted or
refrozen meat was being used.

1.3. I recall being told by Managers that the leftover
shake mix is meant to be thrown away at the end of each
day. However, apparently because this product is
relatively expensive, I recall seeing the shake mix being
removed from the shake machine and stored in the fridge for
use on the next day. This happened almost every night
I was on the close.

1.4. I recall some occasions where the drains at the store
were blocked. As a result, the sewerage would rise and
cover the floor of the back room and kitchen area of the
store. To my knowledge, on such occasions, the store was
never closed to the public; instead the sewerage was moped
out of the sight of customers and the operation of the
store continued as usual.

1.5. I recall out of date lettuce and onions being used in
burgers.

2. Health and safety: 2.1. I recall the hose on the
filtering machine being faulty for a few months. The
management was told about this potential hazard several
times during this period but no action was taken to repair
the machine. One night when I was on a close I was
instructed by Steve Harney, the Floor Manager, to filter
the hot shortening using the machine. The protective visor
was lost and as by then it was very late and we did not
have the time to look for it, I was instructed by the same
Manager to carry out the filtering without a visor. While
I was filtering, the defective hose blew up and the hot
shortening splashed into my face and eyes. I had to be
sent immediately into hospital for a check-up."

That does not appear in the accident book; do you know why
that is?
A. Well, I did not obviously -- first of all, regarding a
general statement about accident book, is that it was not
made clear to the crew members that they should keep their
own accidents in there if that was the case. So, I did not
ever bother to put my accident in the accident book and
nobody else -- obviously, then I was sent to hospital.
I went to hospital. I was too busy to my accident in the
accident book anyway.

Q. "2.2. I do recall the Mac bun toaster's top hot plate
being loose and easily falling on the operator. Indeed, it
happened to myself and I still bear the remnants of the
scar.

I do recall the occasion when Vicky Golding fell on the
grill and burnt both her arms". Sorry, that was paragraph
2.3.

"2.4. Regarding Sidney Nicholson's statement (61),
I recall being officially put in charge of routine
maintenance of the equipment, lighting, etc. at the store
