     2        A.  Yes.

     4   Q.   Was it your responsibility that it should go in there?
     5        A.  It was one of my people that developed it, yes.

     7   Q.   What prompted that development?
     8        A.  It was just one of the things that we felt needed
     9        clarifying.  The accident checklist has been in the
    10        restaurants as long as I can remember.

    12   Q.   I see.
    13        A.  But this clarification of what is a reportable accident
    14        and what is not, we thought it could be laid out a little
    15        better than it was previously.

    17   Q.   Was it in this striking coloured format previously?
    18        A.  It was not, no.  But there are some new safety sign
    19        regulations due out later this year and this format
    20        complies with those as well.  So we thought it was a good
    21        opportunity to get ahead of the things.

    23   Q.   Can we just have a look at a sample or samples of an
    24        incident report form.  They are in tab 57 P of this file,
    25        which is the one before.  It so happens, Mrs. Barnes, that
    26        quite by chance we have these from another source.  They
    27        come from the Glasgow area for the period 93/94 more or
    28        less.  Notice the first one.  It has got slapped on the
    29        front of it "NB: A notifiable accident is" and so on and
    30        then it describes it; do you see that?
    31        A.  No.

    33   Q.   A different order.  Can you find one which has on the front
    34        of it a white sticker?
    35        A.  Yes, I have it.

    37   Q.   Do you know for how long that sort of definition has been
    38        on the front of this book of report forms?
    39        A.  I suspect that this might be something locally that
    40        they have done because I have not seen it before.

    42   Q.   I see.  This would be a local initiative, would it?
    43        A.  Yes, I would suspect so.

    45   Q.   I am right, am I, that these incident report forms come in
    46        books with carbons behind each front sheet?
    47        A.  They do, yes.

    49   Q.   And they are torn off and an incident report form is sent
    50        in and the carbon is left in the restaurant? 
    51        A.  That is right.  There is always a record in the 
    52        restaurant.
