Day 127 - 23 May 95 - Page 36
or enquired was that circuit breakers should be inserted in
the restaurant; is that not right?
A. He did.

Q. Was that done?
A. It was, although he only asked for them to be fitted to
the filter machine sockets. But we later fitted them to
all the circuits in all of the restaurants throughout the
UK.

Q. How long did it take?
A. From memory it took about 12 to 18 months. The newer
stores were quite easy to do because of the technology of
the equipment used. Some of the older stores it was a lot
more difficult both to organise and a lot more costly as
well.

Q. Can you quickly turn to page 780 in the file that is in
front of you, which is dated 13th October 1992 which is the
day after the accident. It is from Paul Preston and signed
by him to all restaurant management. Can you tell us,
please, do you know whether Mr. Preston actually wrote the
words of this document himself?
A. The words of the document were written by Keith Smith
and myself.

Q. And you?
A. Yes, and then taken to Paul by Carmel Flatley, who is
now the vice president, human resources.

Q. Then he says what the immediate steps are that would be
taken in consequence of what happened to Mark Hopkins. Do
you see them there? I am not going to read them out.
A. Yes.

Q. Then at the bottom of the page there is clearly a reference
to RCCB. Then at the bottom of the page, just before the
signature, he says in capital letters: "Ensure that this
information is communicated to all your employees".
A. Yes.

Q. Now, you did an investigation yourself and you will find
that at the back of this file, 57 "S" for Stephen. Have
you got it there?
A. I have got it, yes.

Q. It is dated ----

MR. MORRIS: Which document?

MR. RAMPTON: It is Mrs. Barnes' report of an investigation.

MR. JUSTICE BELL: The draft report which has just been
disclosed.

MR. RAMPTON: It was served last Friday.

MR. JUSTICE BELL: So that has to be 57 S, has it?

MR. RAMPTON: Yes, 57 S. Mr. Stiles, my Lord, has the details
