1 Q. Then there is a recommendation for revising the procedures
2 for cleaning the filtering machines; do you see that?
3 A. I do.

5 Q. "Aim is to better specify procedures so that there is
6 easier access to plugs and minimal use of water". What
7 happened there?
8 A. That was revised by Richard Rowley, our equipment
9 manager, although we still need to continue to use water.
10 Steps were taken to make sure that cables were long enough
11 or plugs were close enough, that we would never again get
12 the taut cable contribution to an accident that happened at
13 Manchester.

15 Q. The next one, "Richard Rowley is working with Ridling to
16 address the weaknesses in the machine highlighted by the
17 EHO" -- that is Mr. Shirkie. Did Mr. Rowley have any
18 success in that exercise, do you know?
19 A. He did. We had retrofits done to the switching
20 mechanisms in all of the existing filtering machines and
21 had the design changed. We were not able to find a UK
22 based organisation that were able to sanction what we had
23 done in terms of giving us some sort of certification.

25 Q. That is the bit you were looking at?
26 A. Yes, that is correct. But since then EC legislation
27 has come in which self-certifies the manufacture -- they
28 self-certify against certain criteria that equipment is
29 safe.

31 Q. The last paragraph on that page, "Mr. Rowley is
32 coordinating setting up the audits"; do you see that?
33 A. I do.

35 Q. Is that done?
36 A. Yes, it has been done. The audit that you saw before,
37 the electrical audit, was amended to include that.

39 Q. So there was in place already an audit and it was added to?
40 A. Yes.

42 Q. Then on the next page 9, "training", "The AEC", you
43 thought, "ought to be revised to include more emphasis on
44 telling people who had been on AEC that they must inform
45 non-qualified personnel that they should not attempt
46 ...(reads to the words)... additional information on
47 wiring a 13 amp plug." Were those things done?
48 A. They were both done, yes, right down to producing
49 circuit boards and part piece of equipment that the
50 students on the AEC would have to show that they could work
51 on properly.

53 Q. Not just drawings?
54 A. No, not just drawings. It was actually a little light
55 circuit board that they have to show they could work
56 properly.

58 Q. The next one, "The floor managers' post package to be
59 revised to better specify what should or should not be
60 done" etc. Was that done?
