13 MR. MORRIS (To the witness): On page 2 of the status report,
14 the third one down under the heading "Dangerous
15 behaviour/condition" -- in fact, all of the points are
16 under "Dangerous behaviour/condition" -- it says on the
17 third one down on page 2: "40 damaged sockets have not
18 been repaired, despite a fixed system check". What is the
19 fixed system check?
20 A. In addition to having all the portable equipment
21 checked, once a year they carry out a check of all the
22 fixed electrical equipment as well. So that would be
23 things like the drinks machine, which, they are not
24 movable, but they obviously need checking as well. What we
25 have instituted since then is if the contractors find
26 anything they are not happy with while they on site, they
27 do not leave until they have fixed it basically, or they
28 leave it in a condition that it cannot be used until they
29 can get back with the parts.

31 Q. So a fixed system check had been done in that store ---
32 A. It just means -----

34 Q. -- but not been acted on when 40 damaged sockets (in the
35 plural) had been found during the fixed system check, but
36 not been repaired?
37 A. Yes, we discussed that earlier on; there certainly were
38 some repairs that had not been done.

40 Q. Just regarding the tree, the only question on that is, in
41 your fault tree, what it is called, in the centre of the
42 page, slightly to the right, it says: "Plug incorrectly
43 wired by crew member". There is no evidence, is there,
44 that the plug was incorrectly wired by the crew member,
45 even if it was in a dangerous condition at the time of the
46 accident, there is no evidence that he had incorrectly
47 wired it, is there?
48 A. It was what I believed to be true at the time, based on
49 the facts, but I understand exactly what you are saying.
