     1        Regional Managers' meetings.  It was a regular meeting that
     2        was scheduled, but I think it was probably before we were
     3        into minutes.

     5   Q.   Would that have been discussed at minutes of the Health and
     6        Safety Management Task Force?
     7        A.  It might not have been, in that once we had agreed with
     8        Andrew we were going to change it, we moved things pretty
     9        quickly.  The problem with the Task Force meetings is they
    10        are only once a quarter, and quite often there are things
    11        that come along that just need to be actioned in between
    12        the meetings.

    14   Q.   Was it discussed at board level, do you know?
    15        A.  I doubt it was discussed at board level, but the three
    16        Regional Managers were all Vice Presidents at the time, so
    17        a part of that executive team anyway.

    19   Q.   Just something that I noticed yesterday, perhaps you can
    20        confirm it:  Notifiable accidents, that does not include
    21        broken bones in the hands or feet, does it?
    22        A.  It does not in a major category, no, but if a person
    23        was off for more than three days then, yes, it would be
    24        reportable, which the likelihood for an injury like that is
    25        they would be off for more than three days.

    27   Q.   You said that any reported accidents are investigated by
    28        the Company.  In the cases where that happened, would there
    29        be a report prepared by the Company, either by yourself or
    30        somebody else in your department, on that accident?
    31        A.  Part of the accidents investigation format is that the
    32        Supervisor or Restaurant Manager, depending on the severity
    33        of accident, takes statements and attaches them to a
    34        covering sheet which just goes through the main points of
    35        accident.  So, if you are asking me is there any
    36        documentation, then, yes, those accidents investigation
    37        reports are kept on file, and are monitored by my people as
    38        well.

    40   Q.   How long has that been done for?
    41        A.  As we touched on yesterday, it was early 1992 when we
    42        first started asking for a formal investigation of
    43        accidents.

    45   Q.   Can I just ask, yesterday you said:  "We do not do any
    46        statistics from incident report forms, but of course the
    47        restaurants do fill them in for minor accidents as well".
    48        When you said "minor accidents", what did you mean by that?
    49          I mean, would not fill out incident report forms for
    50        burns and cuts, would they? 
    51        A.  Some of them do do.

    53   Q.   Some of them do; what, they might report every single
    54        accident that was recorded in the accident book?
    55        A.  Some of them are extremely zealous with their paper
    56        work and would fill out an incident report form for almost
    57        any cut or burn that happened in the restaurant.

    60        A.  No, the common practice is to fall in line with the
