     1        was not hygienic since the temperature of the storage
     2        cabinet was not high enough, specially when defrosted or
     3        refrozen meat was being used.

     5        1.3.  I recall being told by Managers that the leftover
     6        shake mix is meant to be thrown away at the end of each
     7        day.  However, apparently because this product is
     8        relatively expensive, I recall seeing the shake mix being
     9        removed from the shake machine and stored in the fridge for
    10        use on the next day.  This happened almost every night
    11        I was on the close.

    13        1.4.  I recall some occasions where the drains at the store
    14        were blocked.  As a result, the sewerage would rise and
    15        cover the floor of the back room and kitchen area of the
    16        store.  To my knowledge, on such occasions, the store was
    17        never closed to the public; instead the sewerage was moped
    18        out of the sight of customers and the operation of the
    19        store continued as usual.

    21        1.5.  I recall out of date lettuce and onions being used in
    22        burgers.

    24        2. Health and safety:  2.1.  I recall the hose on the
    25        filtering machine being faulty for a few months.  The
    26        management was told about this potential hazard several
    27        times during this period but no action was taken to repair
    28        the machine.  One night when I was on a close I was
    29        instructed by Steve Harney, the Floor Manager, to filter
    30        the hot shortening using the machine.  The protective visor
    31        was lost and as by then it was very late and we did not
    32        have the time to look for it, I was instructed by the same
    33        Manager to carry out the filtering without a visor.  While
    34        I was filtering, the defective hose blew up and the hot
    35        shortening splashed into my face and eyes.  I had to be
    36        sent immediately into hospital for a check-up."

    38        That does not appear in the accident book; do you know why
    39        that is?
    40        A.  Well, I did not obviously -- first of all, regarding a
    41        general statement about accident book, is that it was not
    42        made clear to the crew members that they should keep their
    43        own accidents in there if that was the case.  So, I did not
    44        ever bother to put my accident in the accident book and
    45        nobody else -- obviously, then I was sent to hospital.
    46        I went to hospital.  I was too busy to my accident in the
    47        accident book anyway.
