     3   MS. STEEL:   Were there any steps taken to check all the stores
     4        with similar tiles.
     5        A.  Even before this accident, in September 1990, we had
     6        already given our commitment to replacing all of the sort
     7        of tiles that were involved in this accident.

     9   Q.   They were old-style tiles?
    10        A.  They were the old-style tiles.  We had already given a
    11        commitment to replace them with the Altro flooring, and
    12        doing that in all restaurants over a three-year period.

    14   Q.   Is there now someone on the McDonald's board with
    15        responsibility for health and safety?
    16        A.  Yes, Andrew Taylor.  He took on that responsibility in
    17        late 1991.

    19   Q.   Is that his sole responsibility on the board ---
    20        A.  Sorry?

    22   Q.   -- or his particular responsibility?
    23        A.  I am sorry, I meant 1992.  No, he is Chief Operations
    24        Officer.

    26   Q.   So he is just doing health and safety as part of that?
    27        A.  Yes, he is Executive Vice President, Chief Operations
    28        Officer.  Health and Safety and Food Safety are lead
    29        responsibilities for him.  He comes along to our Health and
    30        Safety steering group meeting.

    33        A.  He takes an active interest in any decisions that need
    34        to be taken between the meetings.

    36   Q.   Do local stores do any analysis of trends in accidents at
    37        all?
    38        A.  Part of what they are required to do, as part of safety
    39        circle meetings, is to go through accidents in the accident
    40        book and monitor any trend.

    42   Q.   So that is a recent development then?
    43        A.  As we discussed yesterday, it is 1992, 1993, that the
    44        safety circles were rolled in.

    46   Q.   Do you know how many prohibition notices get served on the
    47        Company?
    48        A.  It is something that my people monitor now and report
    49        back regionally.  In fact, last year I believe -- two of
    50        the regions that spring to mind, in the North where there 
    51        were no prohibition notices and no other notices, and in 
    52        the Midlands, we just had one improvement notice towards 
    53        the end of year, and that is all that we had.

    55   Q.   What about deferred prohibition notices?
    56        A.  Well, the only one I can remember of those, because
    57        they are a special sort of notice, were the ones served
    58        after Mark Hopkins' death.
