     1        RIDDOR accidents, are they, notifiables?
     2        A.  They are the RIDDOR accidents, yes.

     4   Q.   It mentions the office, the restaurant office accident book
     5        and also the incident report form, you see in paragraphs 1
     6        and 2?
     7        A.  Yes.

     9   Q.   It also mentions in the first unnumbered paragraph the
    10        accident procedure check list which you have just mentioned
    11        I think?
    12        A.  Yes.

    14   Q.   Is that right?
    15        A.  Yes.

    17   Q.   Please turn to page 751 which is a different document.  Can
    18        you tell us what this document is?
    19        A.  I believe this is from the Personnel Policies and
    20        Procedures Manual.

    22   Q.   What is that?
    23        A.  This is a reference manual that all the restaurants
    24        have a copy of which relates, for example, to personnel
    25        procedures on discipline and included in there is a health
    26        and safety section.

    28   Q.   You will have to take this from me, if I am wrong somebody
    29        will in due course correct me.  It appears to contain
    30        virtually the same material as what we saw in the employee
    31        handbook a moment ago?
    32        A.  It would come from the -- the document we could not
    33        read the we had photocopied, the one that goes on the
    34        notice board in the restaurants, that is the reference.

    36   Q.   It is a bit a shorter.  We looked, as you say, at the
    37        health and safety policy statement a moment ago.  The next
    38        topic I want to deal with, Mrs. Barnes, is McDonald's
    39        reporting procedures.  First question, have they changed,
    40        been improved or whatever since you took over in your
    41        present position?
    42        A.  They have stayed very much the same, in that they are
    43        reported by the regional offices.  These days they are
    44        reported by my people or the admin assistants working for
    45        them, but prior to that they were reported by the Personnel
    46        Department.

    48   Q.   Did the serious accidents get back to the Health and Safety
    49        Task Force which is, in a sense, your predecessor in the
    50        same way it does now? 
    51        A.  The statistics were not prepared in quite the formal 
    52        way that we do now, but because there was regional 
    53        representation on the Task Force they would have had people
    54        coming to that meeting bringing along information of any
    55        noteable accidents that had happened.

    58        accident happens or the accidents happen in the restaurant,
    59        tell us how the information gets back to you and what sort
    60        of information it is that you need and receive in order to
