1 MR. MORRIS: We did not even know it had been done until Jill
2 Barnes came into the witness box even though we had applied
3 for documentation on that subject.

5 (To the witness): Do you know of anyone who was
6 disciplined, apart from a colleague of Mark Hopkins, as a
7 result of the failures by the Company -- I think there were
8 15 identified in that internal report -- surrounding that
9 incident?
10 A. Well, I do not know that. I call it disciplined, but
11 as a result of what happened people were instructed to
12 re-examine, look at anything and everything associated with
13 electricity, and certain pieces of equipment, and
14 additional safeguards were put into the operating system at
15 McDonald's, be they equipment, themselves, or electrical
16 supplies into the building.

18 As to our training procedures, we reviewed what was there.
19 I believe we enhanced a few pieces, emphasising once again
20 the dangerous nature of electricity and the care that is
21 necessary. But as for disciplining someone, no, I try to
22 move forward on this and learn from the tragedy to keep it
23 from happening again.

25 Q. There is one document I would like to get out, which is the
26 Health and Safety Update. If you remember, it is the memo
27 of the Health and Safety. I am getting confused. There is
28 the series of memos, which I could not find, and then there
29 was the Health and Safety Update, McDonald's restaurants
30 22nd November 1993 by Jill Barnes. I have not got a
31 reference for it. It may be behind Jill Barnes's
32 statement, I am not sure; Mrs. Brinley-Codd is just
33 checking. I am not sure if it is part of the series of
34 memos. We were going to recall Jill Barnes but if you
35 remember ----
