     2   MR. RAMPTON:  I believe, my Lord, I misheard.  They have given
     3        the wrong references; it is "M" for mother.

     7   MR. RAMPTON:  I thought she said "N".

     9   MS. STEEL:  The figures for 1992, where did they come from?
    10        A.  They would be off the database.  Now, if that year's
    11        one was the year I took maternity leave, so if they were
    12        not complete for the year I was away, I suspect I would
    13        have extrapolated them for the year end, because also this
    14        was prepared towards the end of year of 1993, so
    15        I extrapolated them to the end of the year 1993 as well, so
    16        that is why it has annual equivalent numbers there.

    18   Q.   You were aware by law the stores would still have to report
    19        any RIDDOR accidents?
    20        A.  Yes.  It was still going on.  I delegated it to
    21        somebody in the office to look after the statistics for me,
    22        but she did not keep them quite the way I would have done,
    23        so they were not totally complete.  Of course, it was
    24        before the time I had my three regional hygiene and safety
    25        advisers working for me.

    27   Q.   But when you got back, presumably all the RIDDOR forms were
    28        still, you had not thrown them away?
    29        A.  No, the FT 508s would still have been all kept.

    31   Q.   Would it not have been fairly simple to go through them?
    32        There is only supposed to be between 340 and 350 anyway.
    33        It would not have taken that long to go through them?
    34        A.  I do not know.  I cannot remember back to then.  I know
    35        they were not kept totally as I would have done, and I was
    36        away for six months.

    38   Q.   You said, when I asked about this previously, that you do
    39        have some statistics for that year?
    40        A.  Yes, what I am saying is I would have extrapolated the
    41        ones I had if they had not been complete.

    43   Q.   Did you have any statistics, for example, about the number
    44        of electric shocks that there were
    45        A.  Well, the RIDDOR statistics would be there, yes.

    47   Q.   So is there really any reason why similar sorts of
    48        information could not have been provided for 1992?
    49        A.  Well, apart from the document that was being prepared,
    50        which was to give management a review of the changes that 
    51        had been made since the Health and Safety Executive came in 
    52        to do the assessment, so I did not feel there was any need 
    53        to put all three years down for that.  It was this sort of
    54        information, the information broken down into place that
    55        I had not got, because it was not kept in the same way
    56        I kept it, but for the purposes of this document, which is
    57        a comparative one, I felt it was perfectly adequate.
