Day 127 - 23 May 95 - Page 45
with the Mark Hopkins matter?

MR. RAMPTON: I just want to ask one more question, my Lord, and
then I will stop and it will have done.

THE WITNESS: There are two pages of the chart and one is
missing.

MR. JUSTICE BELL: Mr. Riley has gone to make more copies.

MR. RAMPTON: We will get him to do that over lunch. Two pages
of the chart, but looking at only -- is it headed "status
report"?
A. Yes.

Q. And are there five columns?
A. Yes.

Q. "Dangerous behaviour/condition, remedial action ...(reads
to the words)... date for completion, those involved"?
A. That is right, yes.

Q. What, overall, does this document tell us?
A. Well, it tells us that by the beginning of December we
had really made a big impact on those areas that we had
highlighted we wanted to address. A lot of it builds on
efforts that we had in place before the accident,
particularly with the work that was already going on with
regard to checks on portable equipment and fixed system
checks as well.

Q. So it is, in effect, a progress report; is that right?
A. It is, yes.

Q. One sees it is, as you rightly pointed out, dated 2nd
December, whereas your original report with its memo was
4th November.
A. That is correct.

Q. So this is what you have been able to achieve in the
intervening months; is that right?
A. Yes.

MR. RAMPTON: My Lord, apart from possibly one or two little
bits and pieces, that is the substance of the evidence in
chief. I thought it important to go through that document.

MR. JUSTICE BELL: Yes. We will break off there until half past
two.
(Luncheon adjournment)

MR. RAMPTON: Mrs. Barnes, before we went back to Manchester, in
October 1992, I was asking you about McDonald's structures,
systems, procedures and attitudes to health and safety in
general. I would like to ask you, if I may, now about, in
particular, training in relation to health and safety. For
this purpose you will need volume XIII which you already
have, XI which you already have as well, and another one
called X which is the Operations Training Manual.
